화장을 지우지 않은 채로 잠들면 피부에 좋지 않다는 것은 누구나 알고 있는 사실이다. 그럼에도 유독 피곤하거나 술을 마신 날에는 그대로 잠자리에 눕고 싶은 유혹이 따른다. 화장한 채로 잠들면 밤새 피부에는 어떤 일이 일어날까?
▷파운데이션이 모공 막아=베이스용 화장품이 물리적으로 모공을 막으면 피부에 악영향을 줄 수 있다. 커버력이 있는 파운데이션, 프라이머 등 메이크업 베이스는 피부의 요철과 늘어난 모공을 채워 피부 결을 매끄럽게 보이도록 한다. 그런데 이 화장품 성분이 오랫동안 모공을 막게 되면 모낭 내 무산소 환경이 만들어져 여드름이 생길 수 있다. 의정부을지대병원 피부과 한별 교수는 “무산소 환경에서는 여드름균인 아크네스의 증식이 활발해지고 활동성도 높아진다”며 “이 균은 피부의 면역 세포를 활성화시키고 염증 반응을 일으켜 염증성 닌텐도라이트여드름(뾰루지)을 유발하기도 한다”고 말했다. 모공이 확장될 수도 있다. 화장품이 모공을 막고 있으면 피부 표면이 건조해져 피지를 많이 분비하는데, 그럴수록 모공이 더 늘어나기 때문이다.
색소·향료가 피부 자극=화장품의 색소와 향료는 대표적으로 알레르기 접촉피부염이나 자극피부염을 발생시키는 성분이다. 따라서 피부와의 접촉 시간이 길면 길수록 피부염 발생 위험도 커진다. 심지어 이전에 알레르기를 일으키지 않았던 물질이라도, 노출 시간과 빈도가 증가하면 새로운 알레르기가 생길 수 있다. 색소·향료는 색조 제품뿐만 아니라 파운데이션 등 피부톤을 보정하는 베이스용 화장품에도 초록색, 분홍색 등이 들어있는 경우가 있어 주의해야 한다.
▷아이 메이크업, 색소침착·안질환 가능성= 진한 아이 메이크업을 지우지 않고 자면 눈 주변이 다크서클처럼 어두워지는 색소침착이 발생할 수 있다. 아이라인, 마스카라, 진한 색의 아이섀도 등에는 진한 색소 성분과 함께 피부와 밀착력을 높이는 첨가물이 많이 들어가 있다. 또 눈꺼풀은 전신 피부 중에서도 가장 얇은 피부이므로 같은 양과 시간 동안 색소에 노출되더라도 다른 부위보다 색소침착이 잘 일어날 수 있다. 한편, 눈꺼풀 주변을 깨끗이 닦지 않고 자는 일이 잦아지면 다래끼, 안검염, 충혈 등의 안질환을 유발할 수도 있다. 눈꺼풀에 남아있는 화장품과 노폐물이 눈꺼풀 안쪽의 기름샘인 마이봄샘을 막거나 자극을 줄 수 있기 때문이다. 또한 모공에 남아있는 화장품에 의해 피지와 노폐물이 쌓이게 되면 눈 주위 비립종도 생길 수 있어 주의해야 한다.
자외선 차단제도 자극=자외선 차단제도 닦지 않고 잠들면 피부에 자극을 줄 수 있다. 특히 화학적 차단제는 자외선과 만나면 화학반응과 열 반응이 발생하는데, 이때 생기는 화학 물질이나 열은 피부에 자극을 준다. 화장을 지우지 않고 자는 동안 노출 시간이 길어지면 자극은 더 심하게 일어난다. 한별 교수는 “특히 민감성 피부, 여드름 피부라면 화학적 차단제 사용을 피하고 가급적 외출 후에는 바로 세안을 해야 한다”고 말했다.
▷화장품 세균 감염=화장품에 의해 세균 등 미생물에 감염돼 트러블이 생기는 경우도 있다. 특히 립 메이크업은 입술에 직접 닿고 대부분 오일과 왁스가 포함돼 있어 세균 등 미생물이 번식하기 쉽다. 가급적 개봉 후 수개월 내 사용하는 것을 권장하지만 립 제품은 훨씬 오랜 기간 사용하는 경우도 많으므로 화장을 지우지 않고 자면 감염, 뾰루지 등을 유발할 수 있다.
◇화장한 채 잤다면 마스크팩 도움
아무리 피곤하더라도 화장을 한 지 12시간 이내에는 꼭 깨끗하게 세안하고 자는 것이 좋다. 특히 마스카라나 글리터, 점막에 그린 아이라인 등은 완벽히 지워지지 않는 경우가 많다. 잔여물이 남지 않도록 클렌징 전용 제품을 이용해 꼼꼼히 지우자. 면봉으로 조심스럽게 터치하듯 닦아내거나 이중 세안을 하는여드름피부 것도 방법이다. 어쩔 수 없이 화장한 채로 잠들었다면, 다음 날 피부 수분 보충에 신경 써야 한다. 한별 교수는 “화장을 지우지 않고 잤을 때 가장 큰 피부 변화는 피부 수분 감소로 인한 건조증”이라며 “아침 세안 후 보습 성분을 포함한 마스크 팩이나 토너패드를 피부에 10~15분 정도 올려두면 빠르게 피부 수분도를 높이는 데 도움이 된다”고 말했다. 또 화학성분에 오랫동안 노출돼 피부가 민감해진 상태이므로 다음 날 화장은 최소화하는 것을 추천한다. 기초 화장품도 향, 색소가 없는 제품을 쓰는 게 좋다. 특히 각질이나 블랙헤드 등이 더 눈에 띈다고 느껴 이를 물리적으로 밀어내는 스크럽 제재를 사용하는 것은 절대 금물이다.
얼굴에 올라온 여드름을 짜고 나면 상처가 남는다. 가만히 두면 빨리 낫지 않을 것 같아 집에 있던 항생제 연고 ‘후시딘’을 바르는 사람이 많다. 항생제 성분이 든 연고는 이외에도 ‘에스로반’ ‘박테로반’ ‘베아로반’ ‘바네포 연고’등 다양하다. 여드름에, 또는 여드름을 짠 곳에 항생제 연고를 바르면 정말 여드름이 더 빨리 가라앉을까?
항생제 연고가 여드름 치료에 특출난 효과가 있는 건 아니다. 항생제 연고는 특정 세균에 의한 피부 감염에만 효과적이다. 항생제 연고가 잘 듣는 균과 여드름균은 애초에 종류가 다르다. 여드름을 개선하려면 여드름균을 겨냥해 나온 제품을 사용해야 한다. 시중 항생제 연고를 여드름에 발랐다가 오히려 증상이 악화할 위험도 있다. 일부 항생제 연고는 그람양성균을 억제하는데, 그람양성균 억제 효과가 있는 연고를 바르면 상대적으로 그람음성균이 많아져 이차적으로 모낭염이 생길 수 있다.
가장 큰 문제는 항생제 내성이다. 여드름이 날 때마다 항생제 연고를 사용하다간, 내성이 생겨 항생제를 써도 세균이 죽지 않게 된다. 외상이나 화상을 입어 항생제 연고를 발라야 하는 상황에 약효를 제대로 볼 수 없는 불상사가 생길 수 있다. 얼굴에 넓게 퍼진 여드름에 항생제 연고를 바르는 일이 계속된다면 특히 내성이 생기기 쉽다.
항생제 연고를 용도에 맞지 않게 사용하는 버릇은 피부 알레르기 발생 위험도 키운다.
일반의약품으로 판매되는 항생제 연고 성분은 전문의약품 항생제 연고 성분보다 알레르기 반응률이 높다고 보고된다. 실제로 국내에서 판매 중인 일반의약품 항생제 연고 성분 중 바시트라신, 네오마이신, 폴리믹신은 해외에서 알레르기 유발 성분으로 등재돼 있다.
항생제 연고를 사용해도 증상이 개선되지 않거나, 항생제 연고를 바르고 나서 가려움·부기 등이 생겼다면 사용한 약이 적절하지 않다는 뜻이다. 연고 사용을 당장 중단하고 병원에 방문해야 한다. ▲염증 부위가 단단하고 농이 차 있을 때 ▲통증이 심하고 열감이 있을 때 ▲염증 부위가 코·입과 가까울 때도 일반의약품 항생제 연고를 쓰기보단 바로 병원에 가는 게 좋다.

클렌징은 피부 관리의 기본이다. 피부에 쌓인 노폐물, 먼지 등을 꼼꼼히 씻어내야 맑고 건강한 피부를 유지할 수 있다. 피부 유형별 알맞은 클렌징 방법에 대해 알아본다.
◇건성 피부
건성 피부는 유분, 수분 함량이 높은 클렌징크림, 클렌징로션으로 세안을 한 뒤, 클렌징 폼으로 2차 세안을 하는 게 좋다. 그래야 피부 장벽 손상이 덜하고, 세안이 끝난 뒤에도 당김이나 건조함이 잘 느껴지지 않는다. 각질 제거를 할 때 이마, T존에는 스크럽 제품을 사용하고, 나머지 부분은 피지를 녹이는 로션 타입을 사용하는 게 좋다. 건성 피부는 주름이 생기기 쉬워 가볍게 문지르듯 세안하고, 세안 후 영양크림 등으로 수분을 충분히 공급해야 한다.
◇지성 피부
지성 피부는 피지 분비량이 많아 모공이 막혀 트러블이 잘 생기고, 각질이 많이 쌓인다. 꼼꼼한 세안으로 피지, 각질 제거에 신경 써야한다. 세안 전, 스팀타월을 얼굴에 감싸거나 수증기를 쐬 모공을 열어준 뒤 클렌징을 하면 세정 효과가 높아진다. 피부에 수분을 공급하는 젤 클렌저, 지성 전용 비누를 사용하는 게 좋다.
◇여드름 피부
여드름 피부라면 피부 자극을 최소화하는 게 가장 중요하다. 단, 모공을 막고 있는 각질을 제대로 제거해야 여드름이 생기는 것을 막을 수 있다. 주 1~2회 부드러운 필링 젤을 활용해 묵은 각질을 제거하면 된다. 유분, 자극이 덜한 클렌징 폼, 클렌징 무스 등으로 세안하는 게 좋다. 차가운 물은 피부를 자극하므로 따뜻한 물로 클렌징하고 마무리 단계에서만 약간 차가운 물로 헹궈 모공을 수축하면 된다.
◇매일 화장·면도한다면
매일 화장이나 면도를 한다면 세안할 때 피부 자극을 줄이고, 보습에 신경 써야한다. 메이크업을 지우기 편해 자주 사용되는 클렌징 티슈를 피하는 게 좋다. 클렌징 티슈는 피부에 자극을 주고, 건조하게 만든다. 본인의 피부 유형에 맞는 제형을 사용하되 약산성 제품을 골라 세안하는 게 좋다. 남성의 경우, 면도하기 전 세안을 먼저 하면 부드럽게 면도할 수 있다. 면도 후에는 애프터쉐이빙 스킨케어 제품으로 피부를 진정시키고 수분을 공급해야 한다. 최소 1주일에 한 번은 피부 각질 제거를 해야 면도로 인한 자극, 트러블을 줄일 수 있다.
마스크를 벗고 얼굴을 마주할 일이 많아진 요즘. 그간 가려져 있던 피부 트러블이 고민이라는 사람들이 많다. 특히 인중이나 코에 나는 여드름은 유독 신경 쓰이는 부위로 억지로라도 짜내는 경우가 많다. 하지만 이 부위에 나는 여드름은 일명 ‘안면위험삼각지대(dangerous triangle)’라고 불리며 함부로 짰다간 더 심각한 질환으로 이어질 수 있어 주의가 필요하다.
■안면위험삼각지대 여드름, 심하면 신경손상 유발
안면위험삼각지대에 있는 혈관은 뇌하수체 아래에 있는 해면정맥동과 바로 연결돼 있다. 해면정맥동은 대뇌에 혈액을 공급하는 주요 혈관, 눈의 움직임과 얼굴감각을 담당하는 뇌신경이 지나가는 핵심구조물이다. 따라서 이 부위에 난 여드름을 손으로 짜면 균이 침투하기 쉽고 심하면 뇌까지 퍼질 수 있다.
순천향대부천병원 피부과 이설희 교수는 “안면위험삼각지대에 생긴 여드름을 억지로 짜면 여드름 안에 있던 균이나 손에 있던 세균이 혈관을 타고 뇌로 흘러들어 뇌농양, 뇌수막염을 유발할 수 있다”며 “적절한 치료가 이뤄지지 않으면 혈전을 생성하고 뇌경색, 뇌출혈 등 회복되지 않는 신경손상으로 이어질 수 있다”고 경고했다.
■약산성세안제로 세안, 피부상태 따라 경구제제 복용
여드름을 짜지 않고 관리할 수 있는 가장 좋은 방법은 하루에 1~2회 부드러운 약산성세안제로 세안하는 것. 단 너무 세게 문지르거나 강한 알칼리성비누를 사용하면 피부의 pH(수소이온농도)를 높여 피부장벽을 손상시킬 수 있다.
만일 피부장벽이 손상된 상태에서 로션이나 크림을 사용하면 자극 및 알레르기 접촉피부염이 발생, 이차적인 피부염을 유발할 수 있다. 또 국소여드름 치료제의 자극도 높일 수 있다.
이설희 교수는 “여드름을 스스로 없애려고 하면 치료시기를 놓치거나 여드름흉터가 남을 수 있다”며 “전문의를 통해 여드름균 집락형성을 억제하는 국소항생제 연고를 바르거나 각질로 막힌 모낭을 제거하는 면포용해제를 사용해 치료해야 한다”고 조언했다.
이밖에도 피부상태 및 여드름 중중도에 따라 여드름균의 성장과 염증을 억제하거나 지나친 피지분비를 억제하는 경구제제 복용이 필요할 수 있다. 또 적절한 시기에 피부레이저 시술 및 압출요법을 시행하면 여드름흉터 예방과 치료에 도움이 된다.
이설희 교수는 “안면위험삼각지대가 아니더라도 직접 여드름을 짜는 행동은 ▲세균침투로 인한 피부감염 ▲여드름 부위 염증 ▲색소침착 ▲흉터 등이 생길 수 있어 피해야 한다”며 “특히 농포(고름)가 발생한 경우 적절한 배농요법이 필요해 피부과에서 안전하게 치료받는 것이 바람직하다”고 말했다.

좁쌀여드름은 모공이 막힌 피부 안에 피지, 세포 찌꺼기, 세포에서 만들어낸 케라틴 등이 쌓여 생긴다. 세균 탓에 염증이 생기기 전이라 아프지 않아도, 보기엔 좋지 않다. 내버려두면 붉게 곪으며 염증성 여드름이 되곤 하는 것도 문제다.
◇'맵거나 단 음식' '기름진 음식' 먹으면 피지 분비량 늘어
좁쌀여드름을 완화하려면 피지 분비량을 늘리는 음식을 덜 먹어야 한다. 우선, 지나치게 매운 음식을 피한다. 매운 음식을 먹으면 체온이 오르며 땀과 피지가 많이 분비된다. 열을 발산하려 혈관이 팽창하는 과정에서 피부에 염증을 일으키는 물질이 분비되기도 한다. 피부가 붉어지는 안면홍조가 생기거나 여드름이 심해질 수 있다.
단 음식도 좋지 않다. 혈당 수치를 잘 올리는 식품이 여드름을 잘 유발한다는 국내 연구 결과가 있다. 도넛, 크루아상, 와플, 탄수화물, 컵라면 등 흔히 정크 푸드라고 불리는 음식을 먹으면 피지 분비량이 늘어날 수 있다. 기름기가 많은 음식도 좋지 않다. 삼겹살이나 치킨 등이 대표적이다. 유제품을 지나치게 많이 먹는 것도 여드름을 악화시킬 수 있다.
◇비타민C 풍부한 과일 섭취·운동이 도움
입이 심심할 땐 과일을 먹는 게 좋다. 과일엔 비타민C 같은 항산화 비타민이 풍부하다. 항산화 비타민은 피지샘의 활동을 줄여 피지 분비를 억제해준다. 불규칙한 식사도 여드름 유발 요인 중 하나이므로 끼니는 꼭 정해진 시간에 챙겨 먹어야 한다.
운동도 여드름 완화에 효과적이다. 몸을 움직이면 항염증성 물질 분비가 촉진되고, 혈액순환에도 도움이 된다. 가령, 운동할 때 근육에서 분비되는 마이오카인이란 항염증성 물질은 피부 염증을 억제하고 세포 활성도를 높인다. 단, 지나친 운동은 오히려 피부에 악영향을 미칠 수 있으니 주의한다. 숨이 차서 옆 사람과 대화하기 힘들고 온몸이 달아오를 정도의 고강도 운동을 1시간 이상 하면, 염증을 활성화하는 코르티솔 분비량이 늘어난다.
